Please use this form if your registered group wants to make a change in the information listed in the District 10 registrar database as well as in Fellowship Connection, the General Service Office’s database, which will ensure that you continue to stay connected to your district, to South Florida Area 15, and to the General Service Office. Examples of changes include change in meeting place; change in General Service Representative, Alternate General Service Representative, or group contact; or change in group type (in person, online, hybrid).
